Enter your mom to win. Just fill out this form (one entry per person, please) to nominate your mom for the prize. Children of any age are eligible to register. Nominations will be accepted through midnight Thursday May 6. Winner will be chosen in a random drawing on Friday May 7th at 7:30am and announced live on the air and given 10 minutes to call 386-325-3334 and claim the prize. If the winner or person who entered them doesn't call in, another name will be drawn until the prize is awarded. The winner will receive a day of pampering for herself and 3 friends, including: Breakfast at City Café, a time of pampering at the beauty school salon, including facials, manicures and pedicures for all four ladies, lunch at Musselwhite's, a shopping spree at Angela's of Palatka where the winning mom gets $200 to spend, flowers for the ladies from Ralph's House of Flowers, a chauffeur-driven limousine for the day. The limo will leave from and return to the Hope FM studio. The winner and friends provide their own transportation to and from Hope FM. Prize to be used by the end of May, with the day to be mutually decided on by the winner and prize donors.
